In accordance with Texas Health & Safety Code §181.105 and House Bill 4224, Guardian Behavioral Health, PLLC provides the following information regarding access to health records, licensing information, and consumer complaints.
Requesting Your Health Records
Current and former clients may request copies of their health records maintained by Guardian Behavioral Health, PLLC.
To request records:
Submit a written request through your SimplePractice Client Portal or by contacting our office.
Include your full name, date of birth, preferred contact information, and the records being requested.
Requests may require identity verification to protect confidentiality.
Records will be provided in accordance with applicable state and federal laws.
